Notice on Claim Procedures
- Please send your claim documents to us at the above address within 30 days of the date of the medical receipt so as not to prejudice your rights to claim under the policy.
- For Clinical / dental / Hospitalization and Surgical claims
On the back of the original medical receipt, write down the Policy Number, Name of the Insured (employer) and the Contact Number. - For Hospitalization and Surgical claim
